You'll receive a letter from your surgeon that contains a unique 12-digit Surgeon's Practice ID.

On the day of surgery, get your REALIZE Band Medical Information Card

At the surgery facility, you'll receive a REALIZE Band Medical Information Card with a unique 16-character Patient Card ID.
